In order to process Charges or ReAdmit, ReActivate, or add the Patient from the HybridChart Database in the future, the Patient MUST be linked to a Chart.  See HOW-TO GUIDE – Patients – Understanding Charts for more details about Charts and why they are necessary. If you are not sure whether your Practice has an EHR integration, ask your Administrator.

Link a Patient to a Chart with Existing EHR Integration


1. Navigate to the Queue by clicking on “Queue”.


2. Linked Patients will have a file folder icon under their name. Unlinked Patients will not.  


3. Click the name of the Patient you wish to work on. When their name appears on the right side of the screen, click the CHART tab. 


4. Click SEARCH FOR PATIENT and the system will search for matching Patients in the EHR database. 

If there is a possibility that the name is spelled differently in the EHR, click Change Search Criteria, and search for the correct first and/or last name. 


5. When the correct Patient listing appears below, click LINK


6. When the Patient is linked, you will be able to view and process their Charges.  


Link a Patient to a Chart without Existing EHR Integration


1. Navigate to the Queue by clicking on “Queue”.


2. Linked Patients will have a file folder icon under their name. Unlinked Patients will not. 


3. Click the name of the Patient you wish to work on. When their name appears on the right side of the screen, click the CHART tab. 


4. Click SEARCH FOR PATIENT. No results will populate. Scroll down to CREATE NEW CHART


5. Type an ID number in the orange EHR ID field. You may use the Patient’s EHR ID, MRN, Account Number, or other number designated by your Practice. If you are not sure what ID number to use, speak to your Administrator. Click CREATE AND LINK to link the Patient. 


6. When the Patient is linked, you will be able to view and process their Charges. 


Unlink a Patient


1. If a Patient was linked in error, you may remove the link by scrolling to the bottom of the CHART tab and clicking UNLINK PATIENT.
