The Fields management tool allows you to adjust what Fields are present, active, and required when adding a Patient to HybridChart. NOTE: Some Fields, such as First/Last Name and Hospital are locked and are always required.
1. From the Administration Section, click the Settings View.
2. Click the FIELDS management tool.
3. Set the Fields to Active or Required, as needed, by toggling the button to right to turn on or to the left to turn off.
Fields Definitions
Demographics
· Last Name The patient’s last name.
· First Name The patient’s first name.
· Date of Birth The patient’s date of birth.
· Medical Record Number The patient’s hospital medical record number.
· Account Number The patient’s hospital account/FIN number.
· Race The patient’s race.
· Gender The patient’s gender.
Location
· Hospital/Service The census where the patient is/will be located.
· Floor The floor the patient is/will be located on.
· Room The patient’s room number.
Providers
· Rounder The primary physician rounder for the patient.
· Midlevel The NP/PA rounder for the patient.
· Primary The primary outpatient provider for the patient.
· Resident Rounder The primary resident rounder for the patient.
· Resident Primary The primary outpatient resident for the patient.
· Secondary Rounder The secondary rounder for the patient.
Clinical
· History A free text field that can be used to record a brief patient history.
· Chief Complaint A free text field that can be used to record the patient’s chief complaint.
· To Do List A free text field that can be used to record To Do items for the patient’s care.
· Miscellaneous A free text field that can be used as needed per practice.
· Date of Admission The date the patient was admitted to the hospital/facility.
· Surgery Date The date of the patient’s surgery/procedure.
· Mother Name The name of the patient’s parent/guardian.
· Referring Providers A drop down list of referring providers that is added to the database.
· Code Status The patient’s code status, e.g DNR, Full Code, etc.
· Workup A free text field that can be used to record the patient’s workup.
· Exam A free text field that can be used to record patient exam findings.
· Medications A free text field that can be used to record patient medications.
· Contingency Plan A free text field that can be used to record a contingency plan for the patient’s care.
· Assessment A free text field that can be used to record a brief patient assessment.
Info
· Added The date and time the patient was added to HybridChart.
· Added By The name of the User that added the patient.
· New Patient Auto-checks to indicate that this patient is new. May be unchecked in the Add Patient screen.
· Status The status of the Add Patient process.
· Consult A checkbox that can be checked for patients that are consults.
· Admit Status A drop down menu to select the patient’s admit status, e.g. Outpatient, Observation, etc.
· ICU Patient A checkbox that can be checked for patients who are located in the ICU.
· New to Practice A checkbox that can be checked for patients who are new to the practice.
· Global Payment A checkbox that can be checked for patients on a global payment plan.
· Referral Date The date the patient was referred to the practice.
· Conversion Date The date the patient status converted. Example: Patient was transferred to hospice care.
