The Pathways management tool allows you to add, edit, and delete Pathways.  

Pathways Basics

Pathways is designed to make gathering smart data streamlined and efficient via a series of Triggers and Actions. Once a Pathway is set up, the system will use automatically gather the data. One Trigger may activate multiple Actions, or multiple Actions may be activated by a single Trigger.

Pathways Categories & Triggers

· CHARGE CAPTURE: Charge Capture Pathways are linked to Charges. They are activated by three different Triggers: Charges, Diagnoses, and/or Primary Biller.

· FIELDS: Fields Pathways are linked to checkboxes in the Fields. They are activated by the following Triggers: checking the New, New to Practice, Consult, ICU, or Global checkboxes.

· FLAGS: Flags Pathways are linked to Flags. They are activated by the following Trigger: checking any Flag.

· METRICS: Metrics Pathways are linked to Metrics. They are activated by the following Trigger: selecting any Metric.

· DISCHARGE: Discharge Pathways are linked to Discharges. They are activated by three different Triggers: Disposition, Follow Up Provider, and Follow Up Time.

Pathways Actions

· CHECK FLAG: A designated Flag is checked.

· CHECK METRIC: A designated Metric is selected.

· CHANGE FIELD: A designated checkbox in the Fields is checked.

· SEND MESSAGE: A secure message is sent to a designated recipient.

· MARK FOR DISCHARGE: The patient is Marked for Discharge.

· CHARGE STATUS (NOTE – These actions apply to CHARGE CAPTURE Pathways ONLY): The Charge is set to Received, Hold, Processed, or Retired.


Pathway Examples


Pathways is a powerful tool. Here are some examples of how it can be used:

· TRACK HIGH RISK PATIENTS: Using the Diagnosis trigger, track all patients who are in a specific Diagnosis code set (such as Heart Failure or COPD). Use this information to enroll Patients in Clinics and alert appropriate staff to ensure timely follow-up and reduced hospital readmissions.

· DISCHARGE PATIENTS FROM HYBRIDCHART: Using the CPT codes for Discharge, automatically Mark for Discharge.

· ALERT STAFF OF PATIENT DISPO: Do you need to track Patients who leave the hospital AMA or go to a SNF? Create a Pathway that alerts staff via Secure Message when a Patient is Discharged with a specific disposition.

· TRACK ICU PATIENTS ON CENSUS: Create a Pathway that auto-checks a Flag when the ICU field is selected, making it easy to know which Patients are in the ICU. 


Add a Pathway

1. From the Administration Section, click Pathways.


2. Click the + sign in the Pathway column.


3. Use the drop-down menu to select the Pathway category. Type the name of the Pathway into the NEW PATHWAY box and click ADD.


4. To edit the Pathway name, hover over the name and click the Edit icon. To delete the Pathway, hover over the name and click X.


5. To view a Pathway with its associated Trigger(s) and Action(s), click the name of the Pathway and the Trigger(s) and Action(s) will appear in their respective columns.


Add a Trigger


1. To add a Trigger, select the Pathway by clicking on it.


2. Click the + button in the Triggers column.


3. Select the Trigger Type from the drop-down menu.

· CHARGE: All or part of a CPT code. This Trigger can be a CPT code from the SuperBill, QuickBill, or both.

· DIAGNOSIS: All or part of an ICD-10 code.

· PRIMARY BILLER: The Primary Biller for a Charge.


4. Select the Operator from the drop-down menu.

· EQUAL TO: The Trigger matches the Value exactly. CPT Example: 99205

· STARTS WITH: The Trigger starts with the Value. CPT Example: 992

· ENDS WITH: The Trigger ends with the Value. CPT Example: 05

· CONTAINS: The Triggers contains the Value. CPT Example: 20


5. Type in the Value. You may only associate one Value with one Trigger. Click SAVE AND CLOSE to save the Trigger. NOTE: If you wish to add more Value(s), create additional Trigger(s). If you add more than one Trigger to a Pathway, all Triggers must be met for the Action to occur.


6. To remove a Trigger, hover over the Trigger name and click the X.


Add an Action


1. Select the Pathway you wish to add the Action to by clicking on it.


2. Click the + button in the Actions column.


3. Select the Action Type from the drop-down menu.

· CHECK FLAG: A designated Flag is checked.

· CHECK METRIC: A designated Metric is selected.

· CHANGE FIELD: A designated checkbox in the Fields is checked.

· SEND MESSAGE: A secure message is sent to a designated recipient.

· MARK FOR DISCHARGE: The patient is Marked for Discharge.


4. Select the Variable from the drop-down menu. Click SAVE AND CLOSE to save the Action. NOTE: The Flags and Metrics displayed are customized for your site and may vary from what you see here. You may only associate one Variable with one Action. If you wish to add more Variable(s) to the Pathway, create a new Action(s).


5. To remove an Action, hover over the name of the Action and click the X.
