What is HybridChart? HybridChart is a workflow efficiency and collaboration tool. It’s designed to manage the different parts of rounding on patients in the Hospital or Long-Term Care setting all in one place using cloud-based software. A desktop version is accessed via the web and Providers may download the app from the App Store or Google Play. It consists of five key features: Census Management, Charge Capture, Data Gathering & Analytics, Secure Messaging, and Discharge Planning. All five features work together to create a seamless workflow in the patient care setting.
The Census management piece:
· USERNAME: The USERNAME is a name that is unique to you, the User. Typically it is your first initial followed by your last name followed by a number. Example: Jane Doe would be jdoe1.
· TEMPORARY PASSWORD: The TEMPORARY PASSWORD is for use during your first login, and you will be prompted to update it during the First Time Login process.
· PRACTICE
KEY: The PRACTICE KEY is the unique identifier for your Practice. Typically,
it is an abbreviation of your Practice name. For example, General Medical
Practice would have the Practice Key “GMP”.
2. Review the Service Agreement. If you do not feel that you can agree to the Service Agreement, please stop the login process and contact your Site Administrator. If you agree to the Service Agreement, click I AGREE.
3. Reset your Password into the Password fields and click RESET PASSWORD. Passwords must meet the following criteria:
· At least one upper case letter
· At least one lower case letter
· At least one number
· NO special characters such as !#*
4. APP USERS ONLY: Enter your mobile number and click SAVE MOBILE. Do not include any spaces or punctuation such as ( ) or -.
5. Turn on the appropriate Alerts by toggling the button(s) to the right and clicking NEXT STEP. Alerts will inform you that you’ve received a Secure Message. You may receive Alerts via text message (for app users) or email (for desktop users).
6. Select System Preferences using the drop-down menus and click NEXT STEP. NOTE: You may change Preferences at any time, so if you’re not sure, don’t make any changes now.
· SORT ORDER OF PATIENT LISTS: This allows you to sort Patient lists.
o LOCATION: Sorts by room number
o PATIENT: Sorts alphabetically by Patient last name
DEFAULT QUICKBILL MODULE: This allows you to select a default QuickBill Module. The options available will vary based on your Practice.
7. Providers: Set your Favorite Discharge settings and click LAST STEP. If you are not sure what to put here, you may change or add this later. My Fav DC allows you to preset the most common set of Discharge instructions that you use so you can select them with one click.
· DISPOSITION: Select the most commonly used Disposition from the drop-down menu.
· PROVIDER: Select the Provider that the Patient should follow up with.
· TIME FRAME: Select the Time Frame for follow up.
· COMMENTS: Add any needed comments.
8. Providers: Import the Practice QuickList and click COMPLETE ACCOUNT SETUP. The Practice QuickList is the list of diagnosis favorites designed specifically by your Practice. The button will be defaulted to “ON”; if you do not wish to import the Practice QuickList toggle the button to the left. If you are not a Provider, there is no harm in importing the Practice QuickList.
9. Account setup may take a minute or two to process. Once this is complete, click LET’S GET STARTED and you will be logged into HybridChart.
