How To Log In
1. Navigate to https://evolve.hybridchart.com.
NOTE: All users will log in from the same site, regardless of which Practice they are with. We recommend using the Chrome browser.
2. Enter your Username, Password, and Practice Key in the appropriate fields and click LOG IN.
· USERNAME: The USERNAME is a name that is unique to you, the User. Typically it is your first initial followed by your last name followed by a number. Example: Jane Doe would be jdoe1.
· PASSWORD: The PASSWORD was set by you during your initial login process. The system will force an update every six months for security purposes.
· PRACTICE KEY: The PRACTICE KEY is the unique identifier for your Practice. Typically, it is an abbreviation of your Practice name. For example, General Medical Practice would have the Practice Key “GMP”.
