The Discharge Report displays Patients that have been Discharged from HybridChart, along with their Discharge instructions. It is a functional report, in that it allows the Scheduler/Discharge Planner to indicate that a Patient has been scheduled for the requested follow up by toggling the “Verify” button.  


Discharge Report

1. From the Data Mining section, hover over Discharge and click Discharged.


2. Set the Search Criteria and click RUN REPORT.

 

· Use the CENSUS or ROUNDER drop-down menus to select a specific Census and/or Rounder.

· Use the FOLLOW UP TIMEFRAME drop-down menu to select Patients whose Follow Up is in the specified range (0-3 days, 2 weeks, etc.).

· Use the START DATE and END DATE pickers to select the date range you wish you view.

· Use the SORT BY drop-down menu to sort the results by Patient, Location, Follow-Up Provider, Follow-Up Time Frame, or Discharge Date (oldest-to-newest or newest-to-oldest).

    · If you wish to view Unverified Patients only, click the UNVERIFIED ONLY checkbox.


3. To print or download the report, click PRINT/DOWNLOAD.


4. Click PRINT or DOWNLOAD in the pop-up, as appropriate. Downloaded reports will export to an Excel CSV file. Printed reports will open in a separate tab for printing.


5. Patients who meet the criteria will display in the Patient list. You may access the Patient tabs by clicking on a Patient name.


6. Review the Discharge orders in the DISCHARGE tab. You will see Patient disposition, requested follow up Provider, requested follow up time frame, and any Patient-specific comments. If your Practice has configured Metrics to be used for Discharge planning, scroll down to view selected Metrics for the Patient.


7. When the Patient’s follow up orders have been scheduled, toggle the Verify button to the right to indicate this.


8. If needed, use the NOTES tab to add notes.


9. To adjust the search criteria, click SHOW SEARCH CRITERIA.
