If you click a Census from the Census At-A-Glance on the Patients Dashboard you will automatically be taken to the SuperBill View.


Select a Patient

1. When you navigate to a census list, there will not be a patient selected.

2. To select a patient, click their box on the left side of the screen. You will know which patient you’ve selected because there will be a BLUE line to the right of their name, and their name will also appear at the top of the right side of the screen.

3. If you hover over the patient’s box, several Function buttons will appear.

• The Information button displays the Patient’s History field in a pop-up.

• The Charge button takes you to the SuperBill.

• The Secure Message button allows you to send a message about this Patient.

• The QuickDC button takes you to the Discharge page for this Patient.

• The ReAdmit/ReActivate button allows you to ReAdmit or ReActivate the Patient. NOTE: This only applies to Patients who have been discharged.


Filter Patient List

You may filter the Patient list to make it more manageable.

1. Click the Filter Icon at the top of the list.

2. In the Filter Manager, click the item(s) you would like to filter by. You may filter by Rounder(s), Midlevel(s), and/or Flag(s). When you select the item(s), the FILTER icon will appear to the right of the item(s) selected.

3. To remove a filter in the Filter Manager, click the item(s) and the filter will be removed. You may click the “Remove All” button to remove all filters.

4. Once the Filter has been applied, the top white menu bar will display a FILTER ON indicator, and the Census leaf will turn ORANGE.


Manage Patient Information

Once the Patient is selected, manage their information by clicking on the appropriate tab on the right side of the screen. The tab that you’ve selected will be underlined in BLUE.


VIEW

The VIEW tab allows you to view all the Patient information. You may not make changes to the information from this tab. NOTE: The fields you see on your site may vary based on your settings.

EDIT

The EDIT tab allows you to edit Patient information.

1. Click into the field you wish to edit and type in the appropriate information. The field you are editing will be outlined in BLUE.

2. If you are entering a date, there will be a calendar icon. Click the calendar to select a date from it, or you may simply type in the date.

3. IMPORTANT: There is no Save button. All information is saved once you click outside the box.

SUMMARY

The SUMMARY tab allows you to view all the Charges from the current admission.

1. To view information about a specific Charge, click anywhere on the line for the Charge.

2. A pop-up will appear with the Charge details, including the CPT code(s), ICD-10 code(s), the status of the Charge (Received or Processed), Date of Service, Primary/Secondary Biller(s), and any Modifiers. To close the pop-up, click “Close”.

EHR (NOTE: This only applies for Practices form whom HybridChart is linked to the EHR.)

The EHR allows you to link, unlink, and view EHR information for the Patient. See HOW-TO GUIDE: Patients: Linking EHR for more information.


DISCHARGE

The DISCHARGE tab allows you to view and edit the Patient’s Discharge information. NOTE: Editing Discharge instructions on this tab does not complete the Discharge process for the Patient.


1. Use the drop-down menus and text boxes to enter or edit Discharge instructions. There is no Save button.

2. If Discharge orders are complete for this Patient, you may select the “D” Flag by clicking the box to the left of the Flag. You may also scroll down and select Metrics by clicking the box to the left of the appropriate Metric.

MESSAGES

The MESSAGES tab allows you to view all Secure Messages sent about this Patient. NOTE: You can’t send a Secure Message from this page. To do so, hover over the Patient’s name and click the Message function button.

FLAGS

The FLAGS tab allows you to add or remove Flag(s) for the Patient. Flags are designed to make it easy to know important items about a patient, such as weekend follow up status, at-a-glance from the Patient list. NOTE: Flags are custom configured to meet the needs of your Practice and will vary from what you see here.

1. To add a Flag, click the checkbox to the left of the Flag. The box will be BLUE when the Flag is active.

 Active Flags will appear on the Patient list.

2. To remove a Flag, click the checkbox to the left of the Flag. The box will be WHITE when the Flag is not selected.

DATA

The DATA tab contains Forms, uploaded images, and uploaded documents.

LOG

The LOG tab allows you to track actions performed for a Patient in HybridChart. You will see when the action occurred, what User took the action, and details about the action.
